2026 Community Counseling Program Survey

The Methodist Community Counseling Program (CCP) is seeking your feedback on our program in order to provide the best care to students.

This voluntary survey is being sent to referral sources of the CCP, such as school principals, social workers, and school counselors. Participation is not mandated or required. Please read the questions and provide your opinion of our services based on the rating scale. Submissions are anonymous. Responses will be used by CCP leadership to evaluate referral source experiences and support program improvement.

Results will be shared with Dr. Michael Yoerger in the Research Division. This project is not sponsored by OPS.
