Form 1095-C to be mailed to employees by request only

In late 2024, legislation was announced regarding certain disclosure requirements under the Affordable Care Act. With this legislation, mailing a paper copy of Form 1095-C is no longer required unless requested by the employee. 

Upon request, Methodist Health System will mail the requested IRS Form 1095-C to an employee within 30 days. You can request this form by calling the HR Service Center at (402) 354-2280 and selecting option 1 or by submitting an HR Service Request on the Employee Center. 1095-C forms are additionally available electronically in Workday, where you can save an electronic copy or print it yourself.

About 1095-C:
  • Forms will be available in Workday by March 2026.
  • Employees do not need to wait for their 1095-C form to file their taxes.
  • Employees should keep 1095-C forms with their tax information in case of an audit.
Workday instructions: 
  1. Log in to Workday.
  2. In the search bar, enter “ACA forms.”
  3. Print or download a copy of the 1095-C form.
