At the Methodist diabetes center in Omaha, we can diagnose diabetes and other endocrine diseases or disorders using a comprehensive health assessment and various tests and technologies that:

Patient-Centered Medical Home
Available at many primary care and specialty clinics, our Patient-Centered Medical Home program is a holistic approach to health care that seeks to actively involve patients and families in their health. Our providers, health coaches and other health care professionals assist adult patients in managing the treatment of acute and chronic illness through preventive services and enhanced communication. The program aims to reduce hospital admissions and emergency department visits while improving the health of patients.
