Proxy Access Request

To access another individual's Methodist My Care medical records, please complete the appropriate forms linked below to request proxy access.

Access Someone Else's Medical Records

A proxy access request and authorization form is used to allow access to the medical records of another individual. 

If requesting proxy access to more than one patient, a separate form must be filled out for each request. 

Please allow four business days for processing after receipt of the completed application.

Request Access to an Adult's Medical Records

Submit the form linked below to request access to an adult's Methodist My Care account as the legal representative, person with power of attorney, or as a spouse/other family member.

Submit the form linked below if you are the legal parent or guardian of a minor (under the age of 19 in Nebraska or 18 in Iowa) and wish to access the minor's Methodist My Care account.
